Description
|
Once Mrs. Schnurr receives your registration paperwork, forwarded from the SMCDSB, she will call you to book an appointment. At this appointment you will need to present the following original documents:
Child's Birth Certificate
Roman Catholic Baptismal Certificate (child's or parent's/guardian's)
Proof of Address (Purchase agreement, utility bill, tax bill, MPac assessment document. We cannot accept a lease/rental agreement, cell phone bill or driver's licence)
Completed and signed Application of Direction of School Support form (found on the SMCDSB website)
Completed and signed Separate School Support Lease, if one parent is not catholic (found on the SMCDSB website).
